Cancel an Appointment

Please use this Form to cancel appointments more than 24 hours in advance. If you are unable to attend an appointment please let us know in plenty of time to assign your appointment time to another patient.

Registered for online services?

If you are registered for online services then you cancel the appointment and re-book all at the same time

TO CANCEL YOUR APPOINTMENT, EITHER CALL US ON 01354 656841 OR VIA ONLINE SERVICES.
Appointments that are cancelled within 30 minutes of the appointment time will be marked as DNA (Did Not Attend). 
Arriving more than 10 minutes late for your appointment will result in not being able to carry out the consultation. Another appointment will need to be booked.
